Integrated Community Health invite you to express interest in joining the Community Partnership Committee. Your lived experience is important in co-designing the services we provide, helping us improve healthcare outcomes
Integrated and Community Health (ICH) delivers care in your home or the Community Centre and is a vital part of the service provided by Western Sydney Local Health District. This includes:

Chronic disease management

Child and family care
-
Hospital in the home
-
Aboriginal Community
-
Community Palliative Care
-
Multicultural Health
-
Youth Health
We are looking for people in Western Sydney who want to get involved as a Consumer Representative to bring the patient, client and carer voice to health care services across the Western Sydney Local Health District (WSLHD).
Unfortunately the following applicants WILL NOT be considered for this position:

current health care staff.

applicants with outstanding/pending complaints
-
applicants outside the Western Sydney Local Health District
WSLHD covers Westmead, Auburn, Blacktown, Mount Druitt and Cumberland Hospitals and their local areas only. Unfortunately, we are UNABLE to accept volunteers from areas outside these areas noted above.
If you or someone you care for have experienced a health service in these areas and would like to partner to improve the health system, please contact us. Please note: National Police Checks are mandatory. Meetings are a mix of virtual and face to face and masks may need to be worn.

- **HOSPITALS: Westmead, Auburn, Blacktown, Cumberland and Mount Druitt Hospital (Rouse Hill in development stages)
- COMMUNITY HEALTH CENTRES: Auburn, Blacktown, Doonside, Merrylands, Mount Druitt, Parramatta and The Hills Community Centre**
