HammondCare is passionate about improving the quality of life for people in need. Our Community Palliative & Supportive Care Service provides palliative care and end of life care for patients who wish to remain at home in the Greenwich Community.

Palliative Care patients often find need of social support as their disease progresses. We’re looking for enthusiastic volunteers over the age of 18 to visit palliative care patients in their homes to offer social support. Typically, this will be once per week for between 1-4 hours depending on the needs of the patient.

Visits can include:

• social support for patients living alone
• social outings
• assisting the patient with their shopping
• an opportunity for family members to take a break while you sit with their loved one
• transport to medical appointments

Your impact
