As a Social Worker in this role, you will provide high-quality, person-centered services to individuals at risk of or experiencing homelessness. You will employ a Case Management Framework to assist those who are homeless or at imminent risk of homelessness and with NDIS Cleints

About You:

To excel in this role, you'll need the following qualifications and attributes:

Mandatory:

• 1. Hold a Masters/Bachelor’s degree in Social Work and be eligible for membership with the Australian Association of Social Workers.
• 2. Demonstrate an understanding of Social Work theory and the ability to apply it in practice.
• 3. Ability to work constructively and cooperatively within a multidisciplinary team and collaborate effectively with a wide range of external service providers.
• 4. Proficiency in organization, time management, and prioritization skills.
• 5. Dedication to upholding the YDHS values of care, compassion, respect, integrity, professionalism, and safety.
• 6. Exhibit professional and effective behavior, strong interpersonal skills, and excellent computer literacy.

Desirable:

Experience in NDIS, healthcare or homelessness sectors.
