Assisting older people living in their home connect with their community. This may involve visiting the older person in their home or taking them out in the community to access services, do their shopping or attend appointments. Participation of older people in their community reduces their loneliness, improves their physical and mental wellbeing and provides them with motivation and confidence to undertake their daily living tasks.

• Contact the client to confirm the scheduled visit
• Visit the client in their home for one hour weekly or fortnightly for social contact and friendship
• Ensure caring and courteous communication with the client regarding their health and wellbeing and enhance their social connection
• Report any incidents in relation to the client’s health, welfare or wellbeing to Westmont staff
• Maintain confidentiality of information obtained during the visit unless this information must be disclosed to Westmont to assist the client
• Maintain accurate records and documentation of the visit to the client
• Notify Westmont in advance if unable to make arranged visit to the client
