Volunteer Liaison for the Advance Care Directive (ACD) Volunteer Assist Program
Southern Fleurieu Health ServicesCoordinate the ACD Volunteer Assist program with the support of the Southern Fleurieu Health Service Volunteer Coordinator and Southern Fleurieu Health Advisory Council Committee. Communicate with fellow ACD Volunteers and liaise with Health Advisory Council member via email or phone on a regular basis regarding issues or concerns of the program delivery, upcoming volunteer meetings and supporting issues. Deliver and update as required the educational training program to existing and new ACD assist volunteers (optional). Be aware of any changes to policy and procedures relating to the implementation of the Advance Care Directive program in South Australia and other Australia jurisdictions. Facilitate the uptake of the ACD Assist program by maintaining regular contact with local medical practices across the Southern Fleurieu Region and promote awareness to the broader community by promoting with Service Clubs, Clubs and organisations. Prepare a monthly report for the Health Advisory Council Committee regarding number of community members seen across the three delivery sites and any other issues and concerns. Advise local councils as required (usually monthly) about number of community members in attendance. Comply with policies, procedures of the Barossa Hills Fleurieu Local Health Network Volunteer program.
